Fragment Sheets is a custom homebrew mechanics designed as an overlay for D&D 5e. It activates when characters encounter deeply unsettling phenomena that shake their sense of reality. In these moments, players temporarily shift to simplified Fragment Sheets, representing a fractured state of awareness. These sheets limit their abilities to basic stats, a few core skills, and one cryptic Echo Ability tied to their true self. A shifting Sanity Score governs how much they can retain or reclaim, rising or falling with each strange encounter. It’s a narrative-driven layer that challenges players to endure and recover as mystery unravels.
Centuries ago, something ancient was sealed beneath a remote monastery- neither god nor beast, but a presence so alien it bends the rules of reality simply by existing. It does not speak. It does not dream. It waits.
Two weeks ago, a mining accident cracked open part of its prison. Since then, the region has turned strange. Animals flee the hills. Lights fail without reason. People vanish, then return acting oddly—remembering things they shouldn’t, seeing patterns that don’t exist, watching the sky like they’re waiting for something.
Now, The Order of the Last Binding stirs from its long dormancy. Formed in the final days of the sealing, they were meant to be the last line of defense if the prison ever failed. Silent, scattered, and half-forgotten even among themselves, they now work to recover the four sacred components required to renew the seal.
The ritual must be completed before the solstice, when the veil between worlds is thinnest. As the entity’s influence grows, more of the “changed” begin to gather at the breach. Some are lost. Some are something else entirely.
You are called to investigate the awakening. Whether as a warden, a skeptic, a scholar, or someone caught in the middle- your choices will shape what happens next. Will you help reseal the breach… or let something long forgotten reshape the world?
It is a beginner-friendly Level 3 adventure.
It blends mystery exploration , looming peril, and bit of combat , with plenty of room for character-driven roleplay and storytelling.
